St. Francis Hospital Uses New App to Help Heart Patients Stay on Track

November 1, 2019 鈥 Roslyn, NY 鈥 St. Francis Hospital, The Heart Center庐 has partnered with , a platform for patient engagement, remote monitoring, and virtual care. It is now using the platform for patients undergoing open-heart surgery and transcatheter aortic valve replacement (TAVR) procedures. St. Francis launched the initiative through its affiliation with Catholic Health Services of Long Island (CHSLI) - the first health system in New York to use 无忧短视频鈥檚 digital patient engagement platform, and the first in North America to use the technology for TAVR.

The platform is designed to empower patients and improve care through the course of their treatment. Patients enrolled on 无忧短视频 are provided with step-by-step care pathways (education, reminders, daily milestone tracking) and can track their progress (e.g. pain levels, temperature) via their smartphones, tablets or computers before, during, and after hospitalization. Patients also receive personalized, automated feedback on how to self-manage pre- and post-procedure concerns based on their own inputs. Clinicians have access to alerts and dashboards to monitor patients remotely and address patient concerns earlier, leading to better patient experiences and health outcomes.

To ensure a seamless experience for patients and providers, 无忧短视频 has been fully integrated with CHSLI鈥檚 Epic Electronic Health Record (EHR) and MyChart庐 patient portal. This allows patients to access it through MyChart, while providers can launch dashboards to monitor patient progress and view patient-reported outcomes from within the Epic EHR interface and leverage data to identify quality improvement opportunities.
