How Thunder Bay Regional Health Sciences Centre is Enhancing Rural and Indigenous Patient Care with Digital Care Journey Innovation

At Thunder Bay Regional Health Sciences Centre (TBRHSC), innovation is not just about technology—it’s about ensuring every patient gets the right care at the right time, no matter where they are. By leveraging ǶƵ, a Digital Care Journey Platform, TBRHSC is transforming patient care across a wide range of clinical areas including total hip and knee arthroplasty, hip and knee revision, shoulder, spine, bariatrics, colorectal, thoracic, urology, gynecology, breast cancer, maternity care, and oncology.

A recent case highlights how the Surgical Transitions Remote Patient Monitoring team at TBRHSC is using ǶƵ to overcome communication barriers and provide life-saving care. A post-operative spine patient had returned to their home reservation to recover after surgery. Due to the remote location, poor cell service made traditional phone communication unreliable, with calls repeatedly dropping.

The patient developed concerning symptoms—fever and new-onset neck pain—prompting concerns of possible meningitis. Unable to connect via phone, TBRHSC’s monitoring team turned to ǶƵ’s new Follow Up module, which allowed Katelyn O’Connor, a nurse practitioner on the TBRHSC monitoring team, to communicate directly through ǶƵ with the patient, who was able to open, read, and respond to messages. Acting quickly, Ms. O’Connor instructed the patient to visit the local nursing station for immediate intervention, and additionally coordinated with the patient’s surgeon, local physicians, and nursing stations, ensuring she received critical care without delay. The patient was flown out to TBRHSC and treated for life threatening meningitis, after which she has made a full recovery.

This case underscores a “big win” for the new Follow Up module being used by TBRHSC to bridge gaps in care delivery. By leveraging ǶƵ, TBRHSC ensured that patients—wherever they are—can remain connected to their care team, flag concerns, and receive timely assessment and treatment.

TBRHSC’s proactive use of digital tools demonstrates its unwavering dedication to exceptional patient care. By addressing barriers to access and ensuring equitable care for underserved populations, the hospital continues to set a benchmark for successful healthcare innovation.
